Kusminsky (2007) also suggested that malpositioning is more common with SCV than IJV. The SCV catheterization is often misplaced into the jugular vein up to 15% of the time. Kusminsky (2007) suggested to instruct the patient to turn their head toward the insertion site to compress the jugular vein, thus preventing malpositioning. Contact with the right atrium or ventricle during insertion occurred in approximately 50% of patients and can cause ventricular or supraventricular arrhythmias. Malpositioning has also been associated with local toxicity, perforation, and venous thrombosis (Kusminsky, 2007). Malpositioning of the CVC into smaller veins significantly increases the risk for extraversion, thrombosis, and even cerebral and brain infarction (Polderman & Girbes, 2002, p. 9). Malpositioning of the catheter can cause thrombosis at the end of the catheter, causing it to malfunction, and often requiring replacement of the CVC (Hamilton, 2004).
